does this happen only in those folders? If you add a file in another place, does it show up?

Have you selected a Files saved search in the tree - for example Files/All Files? That is because there is a mechanism in which the scanner which corresponds to the currently selected search in the navigation tree has priority - if you have selected an Outlook search, then Outlook will index with a priority over Files.
